A variety of clinical services are provided by the Florida Department of Health in Walton County & Walton Community Health Center.  Our primary care clinics provide a medical home offering preventative, acute and chronic care for illnesses.

Services include same-day appointments, extended clinic hours, care coordination services, prescription drug assistance and health insurance enrollment assistance, and more.

To participate in our Primary Care Program, a financial interview is needed. 

We accept Medicare, Medicaid, and various insurances.  No insurance?  No Worries! 

We use a sliding scale to determine fees for services.  A sliding fee scale helps to make our services more affordable.  Eligible clients will pay a reduced cost in relation to their income.  Proof of income is required.
