DOH-WALTON UPDATED COVID-19 VACCINATION APPOINTMENT ELIGIBILITY INFORMATION

DeFuniak Springs, Fla. –– Persons 50 or older may now register for an appointment using the Statewide registrations system at https://myvaccine.fl.gov/. Florida residency is required. On the site, residents can select Walton County and submit their contact information. Individuals will be contacted by phone call, text or email and will be assisted in scheduling an appointment. Persons who do not have internet access may call 866-201-3054, TYY number is 833-476-1520.
- Persons 50 or older and critical health care workers
Persons determined by a physician to be extremely vulnerable to Covid 19 (must have letter from physician or completed DOH form found at Walton.flhealth.gov.signed by physician)
Additionally, as part of the statewide preregistration system, each county has a designated
number that individuals can call and pre-register if they do not have internet access. Walton
County’s designated number is 866-201-3054.The statewide preregistration system also provides each county with a designated help line number. This help line will connect individuals directly with an operator and answer questions regarding the preregistration system. Walton County’s help line number is 833-540-2079. DOH-Walton and the Walton County Emergency Management are also working to reach those persons that are homebound and are seeking to get vaccinated. The State of Florida has launched a new way for homebound individuals to sign up to have the vaccine come directly to them. Please email HomeboundVaccine@em.myflorida.com to put in your request.
