Walton County, Fla.— The Walton Community Health Improvement Partnership (WCHIP) is pleased to present the 2020 Community Heart Award to Emily Proctor, Director of Outreach for Caring and Sharing of South Walton. This award recognizes an individual that outstandingly serves Walton County and collaborated with WCHIP to address one or more of the health priorities by supporting community initiatives or activities.

“Emily’s passion for helping those in need is evident by her continuous work to raise awareness of available resources and increase collaboration between community-based organizations, service providers, and the faith-based community,” stated Holly B. Holt, Administrator of the Florida Department of Health in Walton County. “This passion has been a vital community asset during the COVID-19 pandemic and the Mussett Bayou fire in 2020, and her work with Caring and Sharing of South Walton has helped countless families continue stay safe and healthy by providing access to food, financial support, and other resources.”

The Community Heart Award is dedicated to the memory of Gwendolyn (Gwen) Rhodes. Rhodes was an exceptional community partner that represented the Northwest Florida Area Agency on Aging. Her smile and passion for helping all individuals, especially older adults, is a model that WCHIP hopes to honor and carry forward.

WCHIP is a partnership of over 45 community service organizations, local governments, healthcare agencies, and volunteers with a goal to make Walton County a healthier place to live, learn, work, play, and pray. The partnership conducts a community health assessment and creates a three-year community health improvement plan to strategically layout objectives and actions to better the health of all individuals in Walton County.
